Reformat Title on Printing

If you have entered a title as "Blue Box, The" it will print as "The Blue Box"

Reformat Author

An author entered as Lewis, C.S. will show in the body of card as C.S. Lewis

Force Subjects being entered to UPPER case

Whenever subjects are entered, they will be stored in UPPER case.
